These orthotics/insoles may be your first pair – or third, or fourth. Regardless of how many orthotics you have purchased, each of your doctors have probably reassured you that their specific type insole will get rid of or greatly reduce your chronic pain. So you decided – what the heck – I’ll give it a/another shot.
But it’s been a shot in the dark, because in the past, you’ve had no way of determining whether or not your orthotics would be effective and eliminate your chronic pain.
Now you do!
In a landmark paper that I published in the Journal of Cranial Mandibular Therapy, in October of 2013, I radiographically demonstrated that putting an orthotic/insole under your foot can change the position of your cranial bones and first cervical vertebrae.
This understanding becomes very important because it provides an objective way to determine if the orthotics or insoles under your feet will work or not.
This Test Shows You Whether Your Orthotics Will Be Effective And Eliminate Your Chronic Pain
The DORA (Dental Orthogonal Radiographic Analysis) is a radiographic test used by dentists to evaluate the frontal plane position of specific cranial bones. I modified the DORA to evaluate whether specific insoles are functioning correctly.
To have this test done, bring the paper published on my original research Prescriptive Insoles and Dental Orthotics Change the Frontal Plane Position of the Atlas (C1), Mastoid, Malar, Temporal and Sphenoid Bones: A Preliminary Study to your radiologist or a chiropractor who specializes in taking orthogonal radiographs. This paper describes how to take specific Xrays and how to evaluate them.
Below is a general description of what the Modified DORA is.
Using a cephalometric holder (a device which places and holds your head in the correct position), your doctor will:
- take an Xray of your head, while you are standing barefooted (no shoes/no orthotics)
- take a second Xray of your head, while you are standing barefooted on your orthotics.
The doctor then marks specific cranial landmarks on each Xray. Using these cranial landmarks, he takes angular measurements.
Comparing the Xray of standing simply barefooted with the Xray of standing on your orthotics:
- If the sum of the angular measurements are smaller wearing the orthotics than barefooted – this means that your orthotics are working well for you.
- If the sum of the angular measurements are larger wearing the orthotics than barefooted – this means that your orthotics are not working correctly.
In other words, when your orthotics/insoles are appropriate, the angles made by the cranial landmarks are smaller. If your orthotics/insoles are inappropriate, the angles are larger.
*Please note that although my paper talks specifically about proprioceptive insoles used to treat the PreClinical Clubfoot Deformity, this Modified DORA can be used to evaluate all types of insoles or orthotics used for all types of foot structures.
Obviously the Modified DORA holds your practitioner to a higher level of accountability, as he/she can no longer tell you that your insoles are working just fine, even though you still have pain.
So if you are wearing your orthotics, yet you are still in pain, then your orthotics are not effective. I assure you that no matter how effective your practitioner tells you your orthotics are, if they don’t pass my test, they don’t pass the mustard.
